That sounds like a reconstruction job to reduce excess sagging, there is a soft silicone appliance inserted into that vertical incision. Better body shops try not to disturb the nipple if at all possible in order to keep the blood vessels and nerve endings intact. That prevents the random uncontrollable nipple hardening that you see with the cheap saline water balloons that strippers get. It also preserves the girl's nipple sensation, which is definitely a good thing.
