Can someone explain the complete absence of scars?

You just answered your own question. Most don't know what the signs are. Only about 5 of those 400 + reviews were skeptical in the review. The thread on the NY board about 5 years ago was pretty clear that quite a few knew. But many won't acknowledge it for obvious reasons. But similar to here, once it is put out there people acknowledged they had suspicions.
