Active surveillance is good if your Gleason score is a six. Mine progressed quickly to an eight. Some don’t. It could take 10 years to become an issue. My initial PSA was a 15.
Two biopsies a year apart, both showed one core out of 12 with a score of 6. Doc's assessment: "Something else will kill you before this does."

One of the downsides of AS is the need for periodic biopsies to assess for possible progression of disease. This gets to be a real pain (literally) as time goes on. Some centers are now using MRI to evaluate status rather than biopsy

There can be blood in the urine and in the ejaculate after a prostate biopsy
I wish I could send the pic,
I knew about the probable outcome and had a friend give me a handy about a week after the event. She had no idea and it looked looked like a few gun shot blasts of blood. She was like wtf!! Are you ok?
But by a month later or so, all had returned to normal.

First time, about a month after the biopsy was a HJ at an AMP, was like a Quentin Tarantino movie! I'm over 70 and I hadn't produced that much volume in decades. Everything felt normal; actually, very good. Really freaked out the girl at first, until I explained everything; then she was fine.

After watching a steady increase in my PSA numbers for several years, had the biopsy and diagnosed with very early prostate cancer. Decided on the radioisotope seed implant procedure, rather than external radiation therapy, due to not wanting to commit to four or five weeks, five days a week, of traveling and waiting, and exposing more of my body to the radiation. Procedure was fast, almost totally painless, and effective. Due to my age I use 100 to 140 mg of V for L1 or 2, Trimix injections when planning L3. Still works even though not as much volume as it used to be, but still feels good!

At my most recent Urologist follow-up session I found out that the Cialis 5mg daily pills are now available as generic and can be prescribed for BPH. Using GoodRX and a local pharmacy a 90 day supply cost less than $20; can't beat that! So far they work just fine for L1 or 2, and don't have the occasional side effects I had with V, which sometimes caused a stuffed up nose about the time I was face down on the table, though a preventative couple of shots of nasal decongestant prevented that.

If you had very early PCa, were you offered the option of active surveillance, where no treatment is given but there is close monitoring? This seems to be a popular strategy currently, and without the side effects of surgery or radiation

Yes, she explained all my options, but I decided that since I had the actual cancer, not just the rapidly changing PSA numbers, that I needed to do something proactive, not just to wait to "die from something else". I actually had more (temporary) side effects from the needle biopsy than the actual procedure. I'm now two years post, and although my orgasms are of lesser volume and shorter duration, everything still works and is very worthwhile. Thanks for your thoughtful question.

My Uro doc I think is secretly convinced that I have PC but 2 biopsies proved him wrong. My PSA are, and have always been, high. The BPH is a bit of an annoyance but I don't want any plumbing procedures done on my equipment like TURP, or laser etc. I do have difficult keeping it hard even with the "pills" but what I have found is that when I'm with a provider I have a mental problem more than physical with keeping it up. Having performance anxiety about performance anxiety is my main problem. Supposedly, there are studies that show that frequent ejaculations are effective in preventing PC. So I go to erotic spas to maintain my prostrate health. Maybe my insurance will pay for these "treatment" ! Getting old sucks!

I had my prostate removed 20 years ago (cancer). I can still get hard with Viagra and still get the sensation with sex but am shooting blanks (no ejaculation).
I had my prostate removed 14 years ago because of cancer also. They did the nerve sparing robotic route. Same here, can get hard enough with viagra but not as hard as before. Still get the sensation but urine shoots out. Also lost about an inch in length but I'm still around average size.

Weirdly, no. It's a busy practice, these guys are zipping from room to room and shit happens. Maybe I'm too forgiving. But he (and the other docs there) have been overall very good. And I do my homework, so I know what to expect going in. Can't imagine being the poor schmuck who has no idea what his test scores are and ends up under the knife for no reason. I'm sure it happens, just don't want it to happen to me.
The old carpenters saying "measure twice, cut once" would fit perfectly in the medical industry as well.

Was diagnosed with prostate cancer at 56 after my PSA #s started heading northward. Was given the option of surveillance i.e. watchful waiting. Thought about that for a few months and decided I didn't need the stress of always thinking about having prostate cancer. If I were diagnosed in my late 60s might have been a different story.

Had robotic surgery to remove the prostate. Have follow-ups every year since and all is good on the health front. I'm now 64. I can get hard without viagra about 25% of the time but most of the time I need the blue pill. After I take the pill everything works it's just that the orgasms are less intense and shorter lasting. Considering all of the potential alternative outcomes, I can live with that. One thing I've read and heard is that if u had any kind of ED issues prior to surgery then u will most certainly have problems after surgery. Of course the skill of the surgeon is very important, too.

Obviously whether to have surgery is a tough call. I enjoy sex as much as the next monger but making sure I maximize my time on this earth is kinda critical too. Good luck to all those who face this issue. And btw get a PSA test done every year or two. I'm glad I did or else I might not be mongering today.

Hey guys it’s good to get first hand info about this. Having problems emptying bladder after using Cialis for 6 months she scoped me that was a lot of fun!!!
Found prostrate was enlarged a little and bladder was only half empty
She gave me information about euro lift
Or the surgery to clean out prostrate. Just wondering if anyone else has been in this position. O btw gave me other pills to try silodosin side effects don’t sound good.

Yes. I have a hard time getting to full past but can still finish. I frustrates the providers but we work together to get me there and they are often surprised that I finish without being fully at attention.
