Type 'tipping' in the search function for this forum or just about any other. Lather. Rinse. Repeat

For somewhere like epm, not unless you really just think the girl went above and beyond.

For walk-in places, always leave a tip of some sort no matter what, or if you agree on something leave that. Also remember that if you're frequenting a place, tip MMS on the way out. She'll remember you.
