ATB rate structure is almost the same as Cover Girls. It is an additional $300 for FS so I believe the total is either $520 or $540 unless the OP negotiated or offered $500...

It's $$.2 for the hour. Standard used to be an additional $$ for extras with all the fixings. CG is just a little more for their house fee. Have been to both recently (this month) and didn't do more than that. However, there may be some variability with the ladies I suppose.
